Legacy Caregiver Support Program uses an interdisciplinary approach to provide culturally competent resources and support for family caregivers of Medicaid participants with dementia.
The program aims to help caregivers sustainably care for their loved ones, while promoting their own well-being. The program provides family caregiver support,education and training from an interdisciplinary team, and ongoing support, with outcomes expected to include improved caregiver well-being and knowledge of dementia, enhanced coping skills, improved quality of life for the individual with dementia, and reduced healthcare costs.

Our dementia specialists offer person-centered education and training to caregivers to navigate their journey in taking care of their loved ones. Caregivers participate in hands-on activities to equip them with tools to care for their loved ones, allowing them to remain at home as long
as possible. -
During an assessment, OT and PT professionals evaluate the physical capabilities, knowledge, and skills
of caregivers to determine if they can safely assist patients with mobility and activities of daily living. They
also assess the living environment for potential safety hazards or barriers to caregiving and provide education
or referrals for additional training if needed. -
Our RN provides caregivers with training and education on skilled tasks performed in the home, as well as coping skills, such as mindfulness, meditation, yoga, and stress reduction techniques. These practices empower caregivers to manage their physical, emotional, and mental well-being while providing care. Specialized grief counseling is also offered as caregivers experience grief and mourning during the progressive stages of dementia.

Our self-care coordinator collaborates with our team to identify caregivers' needs and barriers to taking time
for self-care activities. They connect caregivers with businesses in our network for self-care services, coordinate respite care, and provide a stipend towards self-care costs. -
Melanin Minded LLC will recognize and serve adolescents who care for loved ones with dementia as personal care workers (PCW). They will receive training, certification, recognition, and potential employment opportunities.

Melanin Minded LLC offers services regarding healthy living to men ages 18 and up in the families in which
we serve that are impacted by their loved one with dementia. They will be enrolled in a program called “Men Making Health a Priority.”
